What you do is put the black web part over the top of a door and then close the door- that keeps the pulley stable and fixed. Now all you have to do is grab each end with each hand, and use your good arm to help hoist your bad arm for a stretch (in many different motions). As you can tell, its just a great way to use your good arm to help stretch out your bad arm.
A few points. With model 240, the pulley will come off each time you open the door. If this bothers you, opt for model 260- it has a metal bracket so the thing stays up even when the door is open.
Also, some people with injuries simply won't be able to grasp the handles strongly enough to keep a firm grip on the handles while using it. Models 260 and 270 overcome this issue by adding a strap around the handle that helps you keep your arm attached to the pulley even if you have a weak grip. Know too that the difference between the 260 and 270 is that the 260 has a metal bracket, while the 270 just uses the webbed strap to hold it in the door. Here's a quick breakdown that might save you the research I had to do to be clear on the differences:
240- regular handle and webbed attachment strap (pulley comes down each time you open the door)
260- regular handle with strap to keep your hand on if you have a weak grip. Also has a metal bracket so pulley stays up all the time.

So get one of these if you're trying to improve your shoulder range of motion. Most people will be using it if they have one limited arm, but you can also use the pulley to increase the flexibility of both arms if you want to.
